Handicapped Parking Tags
Handicapped Parking Tags can be acquired by Town residents at either the Town Clerk's Office, or at the Police Station on Sheridan Drive.

New applicants must provide a completed NY State form MV-664.1 and a driver's license or state issued non-driver ID.  Once accepted, the applications are processed immediately on-site at no charge.

If the applicant is unable to apply in person, another individual can submit the application.  To do so, they must bring the applicants driver's license or state issued non-driver ID, the completed MV-664.1 form, AND their own driver's license or state issued non-driver ID.  The application will be processed immediately on-site at no charge.

To Complete NY State form MV-664.1:

Part 1
needs to be completed in full and MUST include the applicant's signature.

Part 2 needs to be completed by an approved certified medical professional, as outlined in the application.  If a temporary disability is selected, an assistive device MUST be listed.  The bottom of Part 2 MUST be signed by the approved certified medical professional, including their professional license number, overseeing the applicant's care.

Part 3 is completed by the Town Clerk's Office or Police Station attendant.

For Renewals:
If renewing a Permanent Tag, applicants only need to provide the expiring tag and their driver's license or state issued non-driver ID.  If another individual is processing the renewal for the applicant, they must bring their driver's license or state issued non-driver ID, in addition to the applicant's  expiring tag and ID.

Temporary tag renewals are processed on a case-by-case basis.  Please contact the Clerk's Office at 877-8800 ext. 1810, to learn what forms, if any, are needed for renewals or if you have any additional questions regarding parking permits.
